CAMP FORMS & INFORMATION

       The following documents are needed to complete your child’s application:
     

Camp Mulberry Forms/Information

Camper Physical Form – to be completed by a Doctor. The form must be completed within one year of the camp date (within 6 months from the camp date if HIV+).  Campers must complete a TB test within one year of the camp date.

Psycho-Social Statement – to be completed by a mental health professional,  if the camper has seen a Therapist within the past 6 months.

          Immunization history
          Insurance Card   

Please email a copy of your COVID-19 vaccine record (12 and up), immunization record, (which includes a current TB test within one year of the camp date (6 months from the camp date if HIV+), and the above forms to our office. You can email all forms to Samantha at Info@Laurel-Foundation.org

        Transportation Information Directions to Camp Mulberry, drop off and pick up times, and more will  be emailed to you.
