CAMP FORMS & INFORMATION

       The following documents are needed to complete your child’s application:
     

Camp Forms/Information

Camper Physical Form – to be completed by a Doctor. The form must be completed within one year of the camp date (within 6 months from the camp date if HIV+).  

Psycho-Social Statement – to be completed by a mental health professional, if the camper has seen a Therapist within the past 6 months.

          Immunization history
          Insurance Card

• Angel Flight Waiver

  – Only for families with campers flying in on Angel Flight West.

        Transportation Information: Directions to Camp, drop-off, pick-up times, and more will be emailed to you closer to the camp date.
