CAMP FORMS & INFORMATION

Below are the Camper Forms that you need to complete to attend The Laurel Foundation Resident camp programs.

      Camp Mulberry Forms

Camper Physical Form – to be completed by a Doctor. The form must be completed within one year of the camp date.  Campers must have a TB test completed within one year of the camp date.

Psycho-Social Statement – to be completed by a mental health professional. 

Please email all forms and a copy of your COVID-19 vaccine record (12 and up), immunization record, and the above forms to Info@Laurel-Foundation.org. You can also fax the forms to 626-683-0890.

NEW for Camp Mulberry applicants:

In addition to the online application, please submit a 1-2 minute video of your camper answering the following questions:

1. What would going to a camp for gender-diverse youth people mean to you?
2. Why is a camp specializing in gender-diverse youth something you are excited about?

Please email the video to Info@Laurel-Foundation.org and make the subject line: “Camp Mulberry / Camper Name”.  If your child is not comfortable doing a video please have them do a written statement for the above questions and submit.
