CAMP FORMS & INFORMATION

       The following documents are needed to complete your child’s application:
     

Camp Forms/Information

Camper Physical Form – to be completed by a Doctor. The form must be completed within one year of the camp date (within 6 months from the camp date if HIV+).  

Psycho-Social Statement – to be completed by a mental health professional, if the camper has seen a Therapist within the past 6 months.

          Immunization history
          Insurance Card

• Angel Flight Waiver

  – Only for families with campers flying in on Angel Flight West.

        Transportation Information Directions to Camp, drop off,  pick up times, and more will be emailed to you closer to the camp date.
